Individualized Support Services

Community Residential Alternative

We offer a distinctive experience for adults seeking residential care. Our group homes are managed by skilled professionals who provide personalized assistance for daily living needs. Residents will acquire essential life skills within the comfort of their homes, including meal preparation, laundry, housekeeping, money management, and personal shopping. With a small housing ratio of 3-4 individuals per home, we aim to create a secure and homely atmosphere for our residents.

Community Access Group

Our Community Access Services provide adults with special needs the opportunity to engage in meaningful activities and connect with their local community. We offer personalized support to help individuals develop skills, build relationships, and participate in social, recreational, and vocational experiences that enhance their independence and quality of life. Through a variety of programs and partnerships, we foster inclusion and empower individuals to lead fulfilling lives

Community Engagement

Our Community Engagement Program is dedicated to helping individuals with special needs actively participate in their local communities every day. Through meaningful connections and activities, we create opportunities for socialization, learning, and personal growth. Whether volunteering, attending local events, or participating in recreational activities, we ensure that each individual is engaged, valued, and supported as they build lasting relationships and develop a sense of belonging. Our goal is to foster inclusion, independence, and a strong connection to the community for all participants.

Community Living Support & Community Access Individual

We offer customized services to assist adults who prefer independent living or staying at home with their family. These personalized services cater to both individual and family requirements, ensuring support both inside and outside the home. This approach also enables the IDD population to actively participate in the community with personalized one-on-one assistance.

Nursing Services

The Chanan Foundation provides on-site nursing services to support the health, safety, and well-being of individuals receiving services through Georgia’s Department of Behavioral Health and Developmental Disabilities (DBHDD). Our nursing team works collaboratively with individuals, families, caregivers, physicians, and support staff to help ensure each person’s healthcare needs are understood and appropriately addressed
